Can't Find the Power Utility Pak Coupon?

The first 3,000 copies of my Excel 2000 Power Programming With VBA book were inadvertently printed without a Power Utility Pak 2000 coupon.

If you purchased my Excel 2000 Programming With VBA book, you may have noticed that the book doesn't contain a coupon for a free copy of my Power Utility Pak 2000. But don't worry. Purchasing that book still entitles you to a free copy of my software (shipping and handling only). Here's what you need to do:

Print out this page and fill in the blanks
Turn to page 813 in the book, and snip off the page number (a crude form of proof-of-purchase). If you prefer, you can send a copy of the store receipt.
Send me the completed form, proof of purchase, and a check or money order payable in U.S. funds (or credit card information).

You'll receive the software ASAP. I apologize for the inconvenience. Unfortunately, authors have no control over this type of thing.
